This module introduces consequences for substance use during pregnancy, including behavioural reactions, health risks, and social judgment.

Basemental Drugs Compatibility
This module is compatible whether you use Basemental Drugs or not.
If Basemental Drugs is installed: The module will recognize all drugs from Basemental (including tobacco, cannabis, cocaine, MDMA, and more) and apply the full substance risk system to them.
If Basemental Drugs is not installed: This feature will primarily center around alcoholic beverages and the bubble blower from the EA City Living pack. The bubble blower functions similarly to tobacco in terms of pregnancy risks given the insinuation of it being hookah.
Custom CC Drinks Integration
This module is compatible with custom alcoholic drinks from multiple CC creators including Basemental Drugs, Somik and Severinka, Icemunmun, LittleBowBub, Oni, and TianaSims.
Features Common To All Substances
Substance Abuse Tracker
- Each time a pregnant Sim uses any substance, points accumulate on a hidden commodity scale.
- Different substances add different amounts of risk based on their perceived severity.
- If a pregnant sim repeatedly exposes themselves to substance abuse then they increase their risk of their newborn being born with complications (if module 7 is installed) OR premature labour, miscarriage or stillbirth (if Lumpinou RPO or Pandasama childbirth ) is installed.

Warning Tooltips
Before a pregnant Sim uses any substance, a clear warning tooltip will appear explaining why that specific substance is dangerous and what risks it poses to the pregnancy. Different warnings appear for different substance types (alcohol, tobacco, drugs, edibles, psychedelics).
Post Substance Use
After using any substance while pregnant, your Sim will receive buffs that vary based on their personality. Some Sims may feel overwhelming guilt and shame, while others might feel defiant or dismissive about their choices. The emotional response depends on how your Sim feels about their pregnancy and their general attitude toward responsibility.
Social Reactions & Confrontations
When sims witness a pregnant sim using substances, they’ll react based on their relationship, traits, and proximity. These reactions come in the form of confrontation interactions and buffs.
The type of reactions from other sims has been separated based on whether the sims is a stranger, loved one(good friend or family member) or romantic partner to the pregnant sim.
Available Confrontation Interactions
Nearby Sims who notice substance use of the pregnant Sim can then choose various types of confrontation social interactions available in the Friendly, Complaints, and Mean Pie menus.
Supportive Approach:
- Express Concern About Drinking Alcohol While Pregnant
- Gently Question About Smoking Whilst Pregnant
- Confide Worries About Drug Use While Pregnant
- Offer Non-Judgmental Support
Emotional Approach:
- Admit Disappointment for Drinking While Pregnant
- Voice Hurt About Drug Use While Pregnant
Confrontational Approach:
- Berate for Drinking Alcohol While Pregnant
- Lecture About Smoking While Pregnant
- Chew Out for Drug Use Whilst Pregnant
- Yell at for Smoking While Pregnant
Romantic Partner Warnings:
Romantic partners who witness the substance abuse whilst sim is pregnant will trigger warnings to the pregnant sim and put their relationship at risk:
- Initial incidents trigger concern and disappointment
- Repeated substance use leads to romantic partner issuing an Ultimatum
- Continued use after the Ultimatums will trigger romantic partner to break up with pregnant sim
Special Interactions *NEW*
Blackmailed By NPCs
If a pregnant sim uses any substances whilst being in the company of an evil or mean sim, they may risk being extorted for money.
The pregnant sim will have a choice to either pay money to the blackmailer or call their bluff which may or may not succeed.
Reporting Blackmail to Authorities
After the blackmail attempt, the pregnant sim will be able to report the blackmail to the authorities via the Phone >> Household menu.
The police will investigate and call back as to the outcome of the investigation. There will be a chance that the sim will be able to get their money back.
Bartender Bans
Any Sim who witnesses a pregnant Sim drinking alcohol can approach the bartender and request they refuse service to the pregnant Sim. If the request is successful, the bartender will no longer serve alcoholic beverages to that pregnant Sim at that venue. The pregnant Sim will need to go to a different location or find alternative sources for alcohol.
Gossip About Pregnant Sim
Sims who witness substance use can gossip to other sims about what they just saw. Available to any witness regardless of relationship status.
Shame Sim Publicly
This interaction allows sims to publicly humiliate the pregnant Sim for their substance use. This is a harsh, loud confrontation meant to embarrass the pregnant Sim in front of others. Creates significant relationship damage and can trigger public scenes. However not everyone likes a bully so results may vary as for whether the surrounding sims support the pregnant sim or the shamer.
Alcoholic Beverages Specific Features
No additional mods required for this feature, however basemental drugs does enhance it.

- Non-child friendly bar drinks (beer, wine, spirits)
- Ranch Nectar
- Juice kegs
- Moonshine (if Basemental Drugs is installed)
Pregnancy-Appropriate Bar Menu
Pregnant Sims now have access to a separate bar menu with pregnancy-appropriate drinks (mocktails, virgin cocktails, non-alcoholic beverages). This allows them to participate in social drinking without risk.
Certain drinks such as root beer and cream cola are explicitly exempt from the alcohol system. I have also added support for mocktails, including Icemunmun’s mocktails, so pregnant Sims have access to pregnancy appropriate drink options that feel like genuine alternatives rather than just the absence of alcohol.
Tobacco Specific Features
For Basemental Drugs mod.
- Secondhand Smoke Risk: Pregnant Sims near other Sims who are smoking will receive uncomfortable buffs. Importantly, secondhand smoke also increases the pregnant Sim’s substance abuse risk even if they aren’t smoking tobacco directly. It is best to avoid active smokers throughout the pregnancy.
- Bubble Blower (Requires EA City Living Pack): If you don’t have Basemental drugs mod installed, the only tobacco risk that will be in the game is from the Bubbler blower given this is inspired by Hookahs.
- It is not a requirement to have this pack installed unless you want this feature.

This specific feature requires Basemental Drugs mod
Important Note: Autonomous drug-taking interactions have NOT been removed or reduced. Pregnant Sims may still autonomously use drugs based on their addiction status and Basemental settings. This means pregnant addicts may continue using without player intervention unless you actively manage their behaviour.
Substances Included: ALL of the drugs from Basemental are recognized, including cannabis (smoking and edibles), cocaine, MDMA, ketamine, Xanax, Adderall, amphetamines, LSD, shrooms, lean, steroids, and more.
Warning Tooltips – The tooltips differ slightly in this feature for different drugs based on the different levels of risks posed and how well researched it is for its risky effects during pregnancy.
